Health insurance card
Responsibility: Régie de l'assurance maladie du Québec
When a person dies, you must return the health insurance card to the Régie de l'assurance maladie du Québec. Generally, the funeral director will send the card to the Régie in order to simplify the immediate process.
As soon as the Directeur de l'état civil receives the declaration of death, he or she notifies the Régie, which cancels the deceased's health insurance card.
Note: Before returning the card, it is important to take note of the number. You may need it for certain administrative procedures.
The Régie de l'assurance maladie du Québec must be informed of the death of a person insured by the health insurance plan. In addition, the Régie must recover the deceased insured's card in order to cancel it.
